I want to build an on-line questionnaire, a very basic one, are there any ready made applications in the Linux/Open Source world for doing this?

Yes, that's what I've found, "survey" was a better search term than "questionnaire". It seems to be pretty good so an added recommendation gives me confidence to go ahead and try it, thanks. -- Chris Green

Limesurvey (formerly phpsurveyor?) and askpeople are the big two. Limesurvey is high end and askpeople is much simpler. I've used both for work, and surveying add-ons for many CMS/framework systems. Thanks, I hadn't found or heard of askpeople and, although limesurvey looks quite good it does possibly seem a bit 'heavy' for what we want. So I'll take a look at askpeople too. -- Chris Green
